Is there an offline loyalty card app that needs no account?
Yes: StoreCard stores every card in a database on your iPhone and draws the barcode itself, so it works in Airplane Mode and never asks you to sign up. The trade-off is that nothing is copied anywhere else unless you export it yourself.
The blunt way to test it: Airplane Mode on, Wi-Fi off, Bluetooth left on for the Watch, then running through everything the app would normally be used for in a week.

9 of the 13 worked. The 4 that didn't all need a server by nature: ads, the App Store and a web page. None of them stands between you and a barcode at the till.
The Watch Ad one is worth noting. You tap it, the alert closes, and nothing happens: no "you're offline" note, just silence. It costs nothing, since Not Now lets you keep adding cards, but it looks broken the first time.
What "no account" means in practice
There is no sign-up screen, no email field and no login, on the free version or with Pro. Cards sit in a database on the phone that the widget reads too, and the iPhone sends them to the Watch directly over Bluetooth or Wi-Fi. No server of ours ever sees them.
The camera scan and the photo import both happen on the iPhone. When you add a card from a photo, the store name, colours and any member number are worked out on the device; the picture is not uploaded to find them.
The one thing that talks to the internet in the free version is advertising. The banner at the bottom of the list, and the optional video after your third card, load over the network. If you refuse tracking when iOS asks, the ads still show but aren't personalised. Pro removes both.
The catch: there is no backup unless you make one
No account cuts both ways. No server has a copy, so if the phone is lost, reset, or the app is deleted, the cards go with it. There is no iCloud sync either, free or Pro.
Fix: it takes about ten seconds. Settings, then Export Cards, and save the StoCards_Export.json file somewhere that isn't only on this phone. Each card in it carries 17 fields, from the number and barcode format to colours, notes, tags and dates. It is plain text, so anyone who opens the file can read your card numbers. When it isn't the right choice
- You want the same cards to update on two iPhones by themselves. StoreCard can't; you would be exporting and importing by hand.
- Points balances or offers matter to you. StoreCard shows only the barcode and the number on your card, nothing from the shop's own systems, which is exactly why it doesn't need a connection.
- You use an iPad as your main device. It is made for iPhone and Apple Watch.
This shows most clearly at a supermarket where the tills sit in a signal dead spot, or at a gym in a basement. The card opens the same instant it does at home, because there was never anything to fetch in the first place.
